What do we collect, and why?
The enquiry form requests your name and email, with optional date of birth, telephone number, street address, town/city, country, postcode and comments. Required fields are needed to reply. Do not include medical history, identity documents or other sensitive information. We use the details to handle your enquiry and contact you about it. If you separately select the optional marketing checkbox, the practice may also use your name and email for news and commercial information by email. Leaving it unticked does not affect your enquiry. Marketing consent can be withdrawn at any time by contacting the practice.
Optional WhatsApp enquiry updates
If you select the separate optional WhatsApp checkbox, the practice may use your telephone number to send updates and information about your dental enquiry via WhatsApp. This is based on your consent and is separate from email marketing. Leaving it unticked does not affect your enquiry. You can withdraw WhatsApp consent at any time by telling reception or contacting hforge@santaponsadental.com. WhatsApp is provided by WhatsApp Ireland Limited (Meta); use of this service involves sharing your telephone number and message data with WhatsApp. Please review WhatsApp’s privacy policy for information about its processing and international transfers. Please do not send sensitive medical information through WhatsApp.
Legal basis
The proposed basis for general form enquiries and, separately, optional email marketing is your consent under Article 6(1)(a) GDPR, given by actively selecting the form checkbox. You may withdraw this consent by contacting the practice; withdrawal does not affect earlier lawful processing. If your request develops into care or another service, the practice must provide the relevant information and lawful basis separately. No automated decision-making or profiling is carried out by this enquiry form.
Who receives the information?
Enquiries are addressed to the practice reception inbox. Authorised practice staff and the hosting/email providers needed to operate this service may process the information. The planned website host is IONOS; the exact contracting entity, email provider, processing locations, sub-processors and any international-transfer safeguards must be confirmed before launch. Data may also be disclosed where a legal obligation requires it.
How long is it kept?
Enquiries should be kept only for the time needed to answer and deal with the request, with any further retention limited to applicable obligations or claims. Before launch, the practice must confirm its actual enquiry deletion schedule, backup retention, hosting log retention and consent record procedure. Do not treat this draft as confirmation of those arrangements.
Your rights
Subject to applicable conditions, you can request access, rectification, erasure, restriction, portability and objection, and withdraw consent. Contact hforge@santaponsadental.com or write to the practice at the address above. You may complain to the Agencia Española de Protección de Datos (AEPD).
Children and enquiries on someone else’s behalf
A parent or guardian should contact the practice for children’s appointments. Use your own contact details and keep the initial message general. The form is not designed to collect children’s medical information.
Optional Google map
With your permission, the contact-page map connects your browser to Google. Google receives your IP address and other connection data and may use cookies. You can withdraw permission in Cookie settings; this removes the embed but cannot delete Google cookies already on your device. Review Google’s privacy information. The final practice privacy notice must confirm the relevant provider and transfer arrangements before public launch.
Technical data and cookies
Hosting systems may process technical access and security logs. The final provider settings and retention must be verified on IONOS. See the cookie information for the website’s intended use of essential session technology. The private ChatGPT review site has its own hosting and sign-in layer, separate from the planned production site.
Google Analytics — optional
We use Google Analytics 4 (measurement ID G-PVDENZKXJM) only with your consent to measure website visits. Google may process device and browser information and usage data. Analytics cookies such as _ga and _ga_PVDENZKXJM may last up to two years. Advertising signals and ad personalisation are disabled. We do not send enquiry form contents to Analytics. You can withdraw consent in Cookie settings; we then stop tracking and remove accessible Analytics cookies.
